Notice of Privacy Practices

Notice of Privacy Practices

Effective Date: September 28, 2026

THIS NOTICE DESCRIBES HOW MEDICAL INFORMATION ABOUT YOU MAY BE USED AND DISCLOSED AND HOW YOU CAN GET ACCESS TO THIS INFORMATION. PLEASE REVIEW IT CAREFULLY.

This Notice describes the privacy practices of New You Aesthetic with respect to protected health information when the HIPAA Privacy Rule applies.

Your Rights

When HIPAA applies to your information, you have rights regarding your health information. You may ask us how to exercise any of these rights.

  • Get a copy of your medical record. You may ask to see or receive an electronic or paper copy of your medical record and other health information we maintain about you, subject to applicable law.
  • Ask us to correct your record. You may ask us to correct health information you believe is incorrect or incomplete. We may deny the request in some circumstances, but we will explain why in writing when required.
  • Request confidential communications. You may ask us to contact you in a specific way or at a different address.
  • Ask us to limit what we use or share. You may request restrictions on certain uses or disclosures. We are not required to agree to every request. If you pay for a service or health care item out of pocket in full, you may ask us not to share information about that service with your health plan for payment or health care operations when the law requires us to honor that request.
  • Get a list of certain disclosures. You may request an accounting of certain disclosures of your health information for the period allowed by law.
  • Get a copy of this Notice. You may request a paper copy at any time, even if you agreed to receive it electronically.
  • Choose someone to act for you. If you have given someone medical power of attorney or if someone is your legal guardian, that person may exercise your rights as permitted by law.
  • File a complaint. You may complain to us if you believe your privacy rights have been violated. You may also file a complaint with the U.S. Department of Health and Human Services Office for Civil Rights. We will not retaliate against you for filing a complaint.

Your Choices

For certain health information, you may tell us your preferences about what we share. Depending on the circumstances and applicable law, this can include sharing information with family or others involved in your care and sharing information in a disaster-relief situation.

We will obtain your written authorization for uses and disclosures that require authorization under HIPAA. You may revoke an authorization in writing, subject to actions already taken in reliance on it.

How We May Use and Share Your Health Information

Treatment

We may use and share your health information to provide, coordinate or manage your treatment and to communicate with other health care professionals involved in your care.

Payment

We may use and share your health information as permitted to bill and obtain payment for services, products or treatment provided to you.

Health Care Operations

We may use and share your health information for health care operations such as quality assessment, staff training, business management, compliance activities and improving the care and services we provide.

Other Uses and Disclosures

We may use or disclose health information when permitted or required by law, including for certain public health and safety activities, health oversight, legal proceedings, law enforcement purposes, workers' compensation, organ and tissue donation, medical examiner or funeral director activities, and other purposes allowed by HIPAA and applicable law.

We will not use or disclose your protected health information for a purpose that requires your written authorization unless we obtain that authorization.

Our Responsibilities

  • We are required by law to maintain the privacy and security of protected health information.
  • We will notify affected individuals as required by law if a breach occurs that may have compromised the privacy or security of protected health information.
  • We must follow the duties and privacy practices described in the Notice currently in effect.
